How to Become a Patient at Cortex Care Psychiatry
We’re honored that you’re considering Cortex Care Psychiatry for your mental health care. Our goal is to make the process simple, transparent, and supportive from the very first step.
You can start by completing our New Patient Request Form through the patient portal or by contacting our office directly via phone or email.
We’ll ask for a few details about your concerns, availability, and insurance (if applicable) to help us determine the best match for your needs.
Our intake coordinator will review your information and confirm whether we are currently accepting new patients.
If eligible, you’ll receive a call or secure email to schedule your initial psychiatric evaluation.
Once your appointment is scheduled, you’ll receive a secure link to our patient portal to complete your:
- Intake and consent forms
- HIPAA and office policies
- Insurance or payment information (if applicable)
Please submit all forms within 72 hours of receiving them to confirm your appointment.
Your initial visit will last 60–90 minutes, during which your psychiatrist will:
- Review your medical and mental health history
- Discuss your symptoms and treatment goals
- Develop a personalized care plan
For perinatal patients, a complimentary 15-minute pre-assessment may be offered to ensure your needs are well understood before your session.
After your evaluation, you and your psychiatrist will determine the best follow-up schedule for therapy, medication management, or both.

If you have questions about your account or would like to update your payment method, please contact our office directly. To make a secure online payment, locate your Chart ID on your statement and enter it in our online payment portal.

If you’re running low on medication, our team is here to help. Cortex Care Psychiatry strives to process refill requests within 72 business hours.
Please submit it securely through the patient portal. Be sure to include your name, date of birth, medication name, and preferred pharmacy.
Note: Refill requests are reviewed during regular business hours. Controlled medications may require a follow-up visit before renewal.
